Add Print and Save Functions To PDF

I have a PDF which I have added a "print" page function and a "save" page function. The Print function seems to work well, but the Save page I cannot get to work properly. I need someone to fix the Save page function to work properly in most PDF readers. Both of these functions once working properly must be able to be copied by me into numerous other pages compared to the short version PDF I am attaching, thanks.
Please see the bottom of pages 8-9-10-11 of the PDF for the Print & Save buttons.
