dental patient database - repost

I need an Access Database for a Dental Office in Manila. I'm not sure if you realize it, but Dentist in the Philippines earn a very meager income unlike the United States and other developed Western Countries. I'm sure that is true of Australia as well. I found this on Frelancer with your name at the top. I'm not sure how to replicate these into a format that will work with Access 2007. I am trying to help my wife update her business practices in her Dental Clinic. I would appreciate an estimate for building the database in Access and/or Excel. I think Access would be far more productive and easier to tweak or update with new fields, etc. in the future. You would know far better than me. As far as the price and scope of the project I am totally guessing below. Your input is vital in this regard. Thanks in advance! As a developing country, we charge minimum fees for our services, and our profits are nothing like they are in developed countries, for example for an oral exam a patient pays $5.00 or sometimes nothing. An extraction costs $20.00 VAT inclusive. the scope of my practice is general dentistry including, oral examination, extractions, some minor surgeries, filling, basically silver and white composite fillings, removable dentures, basic orthodontics, and maybe a few other simple procedures.
